Submission of application and requirements for MAYOR CIPRIANO D. VIOLAGO, JR. SCHOLARSHIP PROGRAM for the First Semester of S.Y. 2025-2026 is now open.

2. FILL OUT THE APPLICATION FORM USING YOUR DEVICE.

3. DOWNLOAD and PRINT THE TYPEWRITTEN application form on long-size bond paper.

REMINDERS:

* This grant is for COLLEGE STUDENTS only.

* Old scholars should also apply together with the new applicants.

* All requirements will be submitted to the General Services Office (GSO), Ground Floor, Municipal Hall, San Rafael Government Center, Brgy. Sampaloc.

* The deadline for submission is on August 20, 2025.

* Applications with incomplete requirements will not be accepted.

* Details of the qualifying examinations will be announced here on our official page.

REQUIREMENTS:

*For Incoming Freshmen

1. Photocopy of Certificate of Registration (COR) 1st SEM SY 2025-26

2. Certified Photocopy of Report Card – General Average should be 85% above

    (Should be certified by the Señior High School where he/she came from)

3. Barangay Certificate of Indigency (Original Copy)

4. Request Letter addressed to Mayor Cipriano D. Violago, Jr.

    (Personal letter by the student applicant)

*For 2nd YEAR & Up

1. Photocopy of Certificate of Registration (COR) 1st SEM SY 2025-26

2. Photocopy of Certificate of Registration (COR) 2nd SEM SY 2024-25

3. Photocopy of Certificate of Grades (COG) 2nd SEM SY 2024-25

4. Barangay Certificate of Indigency (Original Copy)

5. Request Letter addressed to Mayor Cipriano D. Violago, Jr.

    (Personal letter by the student applicant)
